Services: 

Services included in midwifery care include complete prenatal care, birth and postpartum care (standard maternity coverage through six weeks postpartum). 

Specific Services:


Precious Beginnings Birth Services also referred to as Victoria Stephens, CPM, LDEM in this document, will refer for the following services at an additional financial cost billed by the preferred provider:


Victoria Stephens,CPM,  LDEM, does not provide the following services: 


The Client is responsible for paying for and/or supplying: 


Complete Maternity Care Package $4500

·       Emergency access to the provider 24/7

·       All prenatal appointments, including those that are urgent

·       House calls, when necessary, for urgent/emergent situations

·       19–21-week Anatomy Ultrasound

·       Labor and Delivery support

·       Herbal remedies, and medications, for the process of labor, delivery, and early postpartum

·       Birth Pool and supplies, if desired

·       Immediate postpartum care for the mother and newborn

·       Assistance with breastfeeding, if desired

·       Newborn vitamin k and eye ointment, if desired.

·       Critical Congenital Heart Defect (CCHD) screening for the Newborn

·       Newborn Metabolic Screening

·       Newborn Hearing Assessment

·       Blood typing the newborn, if desired or indicated

·       Any needed Postpartum care in the first 10 days, provided by the provider

·       From 2-6 weeks postpartum care provided by our postpartum provider who offers physical therapy.

·       Specialized Postpartum care plan

·       Filing of the birth certificate and social security card

·       Complementary Gifts will be given each trimester

*Clients that have insurance that will cover the Ultrasound and Metabolic Screening will receive a $300 price reduction.

*If starting are in the 3rd trimester, $250 will be deducted

 

Prenatal and Postpartum Package Only $3500

·       Emergency access to the provider 24/7

·   Placenta Print- $25 if alone, Free with encapsulation service

·   Prenatal Massages with Women’s Health Massage- $400

(10, 60-minute massages. Must be used within 8 months of purchase date)

Payment plan (Complete Care)

At or before 13 weeks: $500 (deposit)

At or before 16 weeks: $450

At or before 20 weeks: $450

At or before 24 weeks: $450

At or before 28 weeks: $500

At or before 30 weeks: $450

At or before 32 weeks: $450

At or before 34 weeks: $450

At or before 36 weeks: $500

At or before 38 weeks: $300


 **If payments are missed, we reserve the right to withhold continued care until payment has been made. If you are late on payment more than 2 times, and another payment plan has not been agreed upon, we reserve the right to transfer your care to another provider and without any refund implied or offered.** 


*If payment is not made in full within 38weeks, or if other arrangements have not been made with the provider, she reserves the right to not attend your delivery, and no refund will be given. If she does not attend your delivery, due to lack of payment, your package will revert to the second package and any remainder will be due within 1 week of delivery. Any late payments will receive a $200/per month fee until paid in full, or another payment plan is agreed upon. Any payment plan for late payments that is not followed, will revert to the original price, and monthly fees will be applied forward from the date of delinquency.

*If the provider is unable to attend your birth due to her own personal reasons, she will be responsible for paying her back-up provider their fees. The client will not be responsible for more than the amount agreed upon below.

Payment Methods:

Precious Beginnings is able to accept cash, check, or venom. All checks must be made out to “Victoria Stephens”. 

Late Fees & Penalties: 

Transfer of Care/Refund

After signing of this contract and prenatal care in initiated, a refund of any moneys will be given based on the discretion of the midwife. If there is a transfer of care for medical reasons, there may be a refund offered for services not performed. For any other discontinuation of care, there will not be a refund given and all remaining moneys owed may be still owed (again, at the discretion of the midwife). Reasons for discontinuation that will not receive a refund include, but are not limited to: moving, change of mind, missing appointments, and loss of pregnancy. Once the Midwife and assistant have arrived at the birthing place, regardless of outcome, no moneys will be refunded. If it is a precipitous birth or the birth team received late notification and misses the birth, no refund will be given. If for any reason you are not satisfied with the services provided, you may write a formal complaint and it will be reviewed, but no refund is implied or guaranteed.  You will at minimum be responsible for payment of the services provided and a $1000 midwife fee.
